Wellness Check: How to Build a Free Scored Wellness Check Quiz

Quick answer

What is a wellness check?

A wellness check is a short questionnaire that rates how a person is doing across a few areas of life, such as sleep, stress, energy and habits. As a scored quiz, it adds up the answers, places each person in a score tier and shows a result that fits them. You can build one for free in FluoTest and start from a ready template.

What is a wellness check?

A wellness check is a quick self-assessment. The person answers questions about areas like sleep, stress, movement, food and mood. Each answer carries a weight, and the total tells you where they stand today.

It is a snapshot, not a diagnosis. For a coach or consultant, the value is simple. You learn who is doing well, who is struggling and who is ready to work with you. That makes your next conversation shorter and more useful.

Why build your wellness check in FluoTest

FluoTest is a free scored-quiz builder. You get unlimited quizzes and responses, all question types, score tiers and routing, and multiple score categories per quiz. That fits a wellness check well, because you can score sleep, stress and energy separately.

You can show a radar results chart when a quiz has 3 or more score categories, and it is free. Branching, custom outcomes, CSV export and integrations such as Brevo and Zapier are free too. Pro, at EUR 12 per month, adds things like badge removal and branded result PDFs. You can start from the wellness check template and edit it.

How to build a wellness check quiz

1. Start from the template

Open the wellness check template preview, copy it into your account and read each question. Keep what fits your clients and remove the rest.

2. Pick 3 or more areas to score

Choose areas such as sleep, stress and energy. Use one score category per area so the results show where each person is strong or weak.

3. Weight the answers

Give each answer a point value. Healthier answers get more points. Keep the scale the same across questions so the totals are easy to compare.

4. Set score tiers and routing

Define tiers, for example needs attention, doing okay and thriving. Send each tier to a different result page or next step, such as a booking link or a free resource.

5. Add branching where it helps

Use conditional branching to ask a follow-up only when it matters. For example, ask about sleep habits only when someone reports poor sleep.

6. Choose a results chart and match your theme

Turn on the radar chart if you have 3 or more score categories. Set your colors, font and font size in the free Theme section of the Design tab.

7. Publish, test and follow up

Take the quiz yourself in each tier. Then share the link and connect Brevo or Zapier, or export a CSV, so you can follow up with each person.
